NOTICE FOR PROPOSED STATEMENT OF QUALIFICATIONS
Design – Build Construction, Salt Storage Building
Sealed proposals with qualifications, in accordance with Osnaburg Township’s request for a design-build project for the construction of a Salt Storage Building for Osnaburg Township at 7115 Hillvale Street SE, East Canton, will be received at the office of the Osnaburg Township Board of Trustees, 7115 Hillvale Avenue SE, Osnaburg Township, Stark County, East Canton, Ohio 44730, until 4:00 P.M., local time, on Monday, April __26_, 2021, when they will be Publicly opened and read aloud. The proposed statement of qualifications envelopes shall be marked “Salt Storage Building for Osnaburg Township”.
On and after Wednesday, March 17, 2021, from 7:30 a.m. to 3:30 p.m., Monday through Friday, plans and specifications are on file for review, and copies of plans and specifications may be obtained from the Osnaburg Township Administrative Office, 7115 Hillvale Street SE, East Canton, Ohio 44730.
The successful applicant shall be responsible for preparing and entering into a contract for the construction of the Salt Storage Building.
Plans and specifications may be obtained from the Osnaburg Township Administrative Office, 7115 Hillvale Street SE, East Canton, Ohio 44730. However, upon receipt of Twenty-five Dollars ($25.00) a copy of the plans and proposed statement of qualifications documents will be forwarded prepaid via ordinary United States Mail. The Township is not responsible if proposed statement of qualification documents mailed are received too late for filing a timely proposed statement of qualifications. Checks for a copy of the plans and specifications shall be made payable to the Fiscal Officer – Osnaburg Township Board of Trustees.
Each proposed statement of qualifications shall be in conformity with the Revised Code of Ohio and shall contain the names of every person and company interested therein.
Each proposed statement of qualifications shall be accompanied by a proper certificate by corporation not chartered in Ohio, testifying that such corporation is authorized to do business in Ohio.
Each proposed statement of qualifications shall be accompanied by a proposed statement of qualifications guaranty in the form of 1) a certified check, cashier’s check or letter of credit, equal to 10% of the proposed anticipated construction contract based upon the statement of qualifications; or 2) a proposed statement of qualifications guaranty bond for the full amount of the proposed construction contract, both forms of guaranty as provided in Chapter 153 of the Ohio Revised Code and in the Project Specifications. The amount of the proposed construction contract for said guaranty purposes shall include all additive and deductible alternates. All proposed statement of qualifications guaranties shall be conditioned that if the proposed statement of qualifications is accepted within sixty (60) days of the proposed statement of qualifications and to entering a contract shall be executed and secured by an approved Performance Bond for one hundred percent (100%) of the contract; and in event of default said bond or check and the amount represented thereby shall be forfeited to the Osnaburg Township Board of Trustees as security.
Proposed statement of qualifications shall comply with the Affirmative Action – Equal Employment Opportunity Requirements set forth in the specifications. A proposed statement of qualifications/contractor will be deemed committed to the goal of such proposed statement of qualification’s conditions by submitting a properly signed proposed statement of qualifications. Each proposed statement of qualifications must ensure that all employees and applicants for employment are not discriminated against because of race, color, religion, sex, handicap, or national origin. Construction work shall be subject to the prevailing wage rates established by the Ohio Department of Industrial Relations (see specifications for details).
Terms of payment shall be as provided in Chapter 153 of the Ohio Revised Code.
No work shall begin prior to authorization in writing by the Board of Osnaburg Township Trustees and total project be completed NO LATER THAN October 30, 2021.
The Osnaburg Township Board of Trustees reserves the right to reject any or all proposed statement of qualifications, to waive any informalities or irregularities in the proposed statement of qualifications received and to accept any proposed statement of qualifications or combination of proposed statement of qualifications which is deemed most favorable to the township at the time and under the conditions stipulated. The Board further declares that they will award the contract for this project to the lowest responsible proposed statement of qualifications, which may not necessarily be the lowest proposed statement of qualifications. No single factor will control the Board’s decision to award, and the Board reserves the right to exercise its full discretion.
